About Hospice

The Yolo Hospice Staff

Hospice is about how you live.

It is a unique healthcare option available to those with life-limiting illness, emphasizing quality of life.

It is about living life to its fullest, up to the final moment. It is about receiving comfort and care when a cure is no longer possible.  It is about surrounding yourself and your loved ones with medical, emotional and spiritual support.

The key to hospice is the well-rounded team approach to addressing the needs of patients and family. Our interdisciplinary team develops a care plan that addresses each patient’s individual physical, emotional and spiritual needs. Nurses, home health aides, spiritual specialists, bereavement counselors and social workers are all part of the team. Trained volunteers may also offer companionship. Our approach ensures symptom control, pain-management, psychosocial support and spiritual care to patients and their families.

Emotional and spiritual support is available for families through the bereavement process and after the death of their loved one.

Hospice is a not a place, it is a program. Care takes place wherever anyone calls “home.” Whether in a personal residence, a skilled nursing facility, assisted living facility or a motel, we provide care to the patient there. Hospice services are available to patients of any age, religion, race or illness.

We often hear people say, "We wish we had known about Hospice sooner." We wish they had too! When someone is confronted with a life-threatening illness Yolo Hospice has a professional staff with the knowledge, experience and concern to help.
