Community Outreach

In addition to caring for patients, Yolo Hospice provides community services and sponsors several community events.

Advance Health Care Directive Program—Who Will Speak for You?

Most people are not prepared when they face difficult medical decisions. An Advance Healthcare Directive is a legal document you fill with your healthcare wishes in writing prior to the occurrence of a medical crisis.

We are available to meet with community groups, government agencies and businesses to show you how to create your own Advance Directive. You may also contact Yolo Hospice to request an Advance Directive form to fill out on your own.

• Select an agent — someone (and at least one alternate) who knows your values and you trust to speak for you if you are unable to communicate your wishes.

• Discuss your wishes with your agent, alternate agent(s), physicians, and those important to you.

• Give copies of your completed Advance Health Care Directive to your agent, your alternate agents (if any), your physician and your loved ones.

• Periodically review your Advance Health Care Directive to make sure it still reflects your wishes and it is valid in the state where you live.

Speaker's Bureau

Through our educational outreach program, we provide speakers to service clubs, religious organizations, community programs, agencies, senior groups and many others. We have a variety of subjects to talk about from “Hospice Care 101”, to “Filling out an Advance Directive”. Call us and we will tailor a speech for your audience.
