Patients on hospice have many different illnesses, but all are reaching the end of life and have a terminal illness.  When a patient transfers to hospice care, the Yolo Hospice Medical Director serves as a consultant to the patient’s attending physician for any pain or symptom management issues. The patient may choose their attending physician at the time they become a hospice patient. Many chose their primary care physician or the specialist treating their illness such as an oncologist.

Registered Nurses from Yolo Hospice make regular visits to the patient, which include assessment of the terminal disease process and management of the symptoms of the disease. They are highly trained in teaching skills to the caregiver(s) and addressing end-of-life issues. The Registered Nurse coordinates the hospice team as they care for the patient. On-Call Registered Nurses are available after office hours for needed visits 24 hours a day, 7 days a week.
