Getting Involved

Burn your Advance Directive
to a Mini-CD

After you have completed your Advance Health Care Directive, you can mail or bring a copy to the Yolo Hospice office in Davis. We will scan it, and burn it onto a mini-CD.

This CD is small enough for you to carry in your purse or wallet.

We will send ALL materials back to you. Consider requesting duplicate CDs, as you may wish copies to go to your agent, physician, or local ER.

This is a free service provided by Yolo Hospice.
