Social Work

Yolo Hospice's top priority is caring for the patient. Care giving is more than medical care. Patients as well as their families frequently need assistance with other non-medical matters related to an illness. Supporting the entire family unit and caregiver(s) in these matters is a crucial part of our work.

Social worker visits enhance the patient’s and family’s lives through support and counseling on end-of-life issues. This may include someone to counsel them about insurance benefits, remind them about living will, trusts, or durable power of attorney. Social workers can also assist with advance directives or funeral arrangements. Our medical social workers are trained and experienced in helping with these matters or can make a referral to other professionals with more specific expertise.

Death and dying can translate directly into loss of control, fear and uncertainty. In the midst of these changes, social workers provide structure and empowerment. They have the dual role of providing emotionally supportive counseling as well as guidance around business, resources and final planning issues. They meet the patients and families exactly where they are, whether in turmoil or tranquility, helping to carry them through the natural progression of dying and grieving, with assurance that their concerns have been met.
